I tested this device and I use it with 3CX on IIS 7. I installed Windows 7 on this. Tweaked it yes. But what can you expect with 1GB of ram. Win 7 uses 35-40% of it - but it is still ok. XP uses the same so why not go for a later OS? I could make around 25-30 sim calls with 3CX. Testes 3CX Only so I don't know how it performs on other OS's. Good device. Switch on RDP, and tweak it to bare necessities. Very low on power consumption. Also mine has wireless inbuilt. Perfect mobile pbx. Can be used for small office yes. Goes to some sort of "Sleep" mode when idle for a period of time which will really affect voip calls when it gets suddenly hit with a call. Regains performance quickly. This should be mentioned in the specs of the device. This is how atom uses less power. By putting the os to sleep when unused. Keep it busy and it will not be a problem. I use it as a mobile pbx when I want to travel and set up my small office with IVR and Voicemail in mobile mode.Overall for serious production use - no go. For individual users that like to have their own personalized phone system its great. I like it.

You can do either. For connection to regular phone circuits you just need a gateway device like ones made by Patton or Audiocodes.

Do VOIP PBX systems, like the one described here, require VOIP technology service providers? In other words, it won't work over normal Bell Telephone copper wires, right?

Any idea how many concurrent lines this would support? I've been looking for something like this, curious what size office it could support.

It depends on what all you are asking the system to do. With trixbox CE, it should easily be able to handle 20-25 calls. With 3CX it should probably be able to do the same unless users are all using the 3CX Assistant, in that case I would say more like 6-8. With only 1mb RAM and it not being expandable, I would really try to aim it at the smaller installs.
